Education Facilitator
The education facilitator serves as a liaison between families and school administrators and staff to promote understanding of school and education system policy and the rights and responsibilities of parents, students and administrators with respect to the design and delivery of special education services for the student.
The facilitator will be expected to address issues related to school curriculum and the school environment and will help develop and maintain partnerships between FHF and education organizations and agencies, including conducting presentations and workshops, and also providing information to agencies, organizations and the general public regarding academic, vocational and social issues relative to students with disabilities. The education facilitator will attend individualized education program (IEP) meetings and case conferences as necessary, and communicate both in person and by telephone with parents and administrators. Individuals who have experience with education issues, special education law and school system procedure and policy are encouraged to apply.
CSHS Parent Liaison
The parent liaison is responsible for assisting parents of children and youth with special health care needs who are consumers in the Children’s Special Health Services (CSHS) clinics. The parent liaison will work as a member of a team with CSHS personnel, including nurses, social workers and clinic staff in providing peer-to-peer support, information about local and regional services, and referrals to appropriate sources of assistance. The liaison will also participate in regularly scheduled statewide, regional, and local trainings, and will assist both the CSHS and FHF teams in conducting outreach to consumers, physicians and service providers in the region. Parents or family members of children and youth who are or have been in the CSHS clinic system are encouraged to apply.
